BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/bcr-2014208787 DESCRIPTION A middle age man was admitted to our infectious diseases department with recent weight loss, fever, dyspnoea and palpitations. Blood and urine tests showed only increased C reactive protein and mild anaemia. As the patient’s wife was previously diagnosed with pulmonary tuberculosis (TB), we performed an interferon γ assay (QuantiFERONTB-2), which turned out to be positive. Sputum specimens revealed the presence of Mycobacterium tuberculosis, and TB chemotherapy (isoniazid, rifampicin, pyrazinamide and ethambutol) was immediately started. A chest and abdomen CT scan could not detect pulmonary lesions, but revealed multiple masses infiltrating the right atrium and ventricle up to the outflow tract. A few days after admittance, the patient developed heart failure because of reduced right ventricle inflow, visualised by transoesophageal echocardiography and MRI (figure 1), associated with atrial fibrillation with fast ventricular response. He was transferred to the intensive care unit for mechanical ventilation, haemodynamic support with fluids and amiodarone for rate control; he eventually required urgent cardiac surgery for repair of the inflow obstruction. The histological examination of the cardiac mass resulted positive in the Ziehl-Neelsen staining, and the diagnosis of cardiac tuberculoma was made. Recovery from cardiac impairment was slow, and the patient received antitubercular chemotherapy for a total of 6 months. At 9-month follow-up, he did not require further antiarrhythmic drugs and the cardiac MRI (figure 2) showed a single residual chronic intramural non-active lesion, without any effect on cardiac filling.

Cardiac tamponade as a presenting symptom of bronchial carcinoma.
There are many causes of pericardial effusion. After excluding lymphoma and leukaemia, cardiac, metabolic, infective or immunological disorders take precedence to malignancies in the differential diagnosis. Clinical pericardial effusions are uncommon in patients with carcinoma of the bronchus, and cardiac tamponade as a presenting symptom is very rare. We report three such patients and relate t...

متن کاملBloody pericardial effusion in patients with cardiac tamponade: is the cause cancerous, tuberculous, or iatrogenic in the 1990s?
STUDY OBJECTIVES The decrease in incidence of tuberculosis, along with the increase in invasive cardiovascular procedures, may have changed the frequency of causes of bloody pericardial effusion associated with cardiac tamponade, although this is not yet recognized by medical textbooks. We analyzed the causes of bloody pericardial effusion in the clinical setting of cardiac tamponade in the 199...
متن کاملTuberculous pericarditis leading to cardiac tamponade: importance of screening prior to immunosuppression
Mycobacterium tuberculosis (TB) presenting with pericardial disease complicated by cardiac tamponade is rare in the developed world, although it occurs more frequently in the context of immunosuppression. In this report, a 74-year-old man on methotrexate for rheumatoid arthritis presented with fever, productive cough and cough-induced syncope. During his admission, he developed clinical signs o...
متن کاملTuberculous Pericarditis Presenting as Multiple Free Floating Masses in Pericardial Effusion
Pericarditis is a rare manifestation of tuberculosis (Tb) in children. A 14-yr-old Korean boy presented with cardiac tamponade during treatment of pulmonary tuberculosis. He developed worsening anemia and persistent fever in spite of anti-tuberculosis medications. Echocardiography found free floating multiple discoid masses in the pericardial effusion. The masses and exudates were removed by pe...
متن کاملCardiac tamponade mimicking tuberculous pericarditis as the initial presentation of chronic lymphocytic leukemia in a 58-year-old woman: a case report
INTRODUCTION Chronic lymphocytic leukemia is an indolent disease that often presents with complaints of lymphadenopathy or is detected as an incidental laboratory finding. It is rarely considered in the differential diagnosis of patients presenting with tamponade or a large, bloody pericardial effusion. In patients without known cancer, a large, bloody pericardial effusion raises the possibilit...
